Family Health Team providing primary care in Haldimand for over 15 years

HALDIMAND—The Haldimand Family Health Team (HFHT) was established in 2005 as a primary health care    organization, along with a number of other family health teams in Ontario. At the HFHT, an interdisciplinary team of health care practitioners works collaboratively with physician practices to provide patients with primary care and health promotion throughout the community.
